FORM delivers medical treatment of obesity through telehealth.

Obesity impacts more than 40% of the US adult population today and will impact more than 50% by 2030. While historically only ~1% of patients received appropriate medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth.

FORM is on a mission to provide compassionate, accessible, evidence-based clinical care, ending the biology vs willpower debate. FORM’s accessible virtual clinic ensures evidence-based care -- including GLP-1s -- are available and leveraged appropriately to deliver long-term outcomes at manageable costs for patients and their financial supporters (employers, healthcare plans, etc).

FORM patients work 1:1 with an obesity medicine Doctor and Registered Dietitian to achieve their weight loss and health goals. The FORM Care Team creates a personalized plan that addresses lifestyle change and includes a prescription for FDA-approved medication, if appropriate. All care is provided virtually, making it accessible to everyone. Patients use the FORM app to meet with their Care Team for frequent video visits, message their Team between visits, track weight, and engage in educational resources. FORM is covered by most major insurance plans, including Medicare.

We launched more than four years ago on iOS and Android, and are looking for an experienced engineer to join our small but nimble team.

You’ll report to our Director of Engineering, helping to build a world-class product, service, and team. Expect to have a ton of influence and impact!
  • Develop back-end services and clinical tools with Ruby on Rails, Sidekiq, GraphQL, and ReactJS, used by physicians, nurse practitioners, registered dietitians, clinical admins, schedulers, billing specialists, and more
  • Collaborate with cross-functional teams to support rapid iteration of internal tools
  • Help our business operations scale efficiently and create better experiences and outcomes for our patients
  • Take ownership of infrastructure (AWS), API services (REST, GraphQL), data modeling (PostgreSQL), integrations, and other services
  • Support experimentation and rapid iteration of a mobile patient telemedicine experience
  • Collaborate with other developers: native mobile, web, back-end/full-stack, and data engineers
  • Inform technical, product, process, hiring, and architectural decisions (including build vs buy)
  • Drive continuous improvement in bi-weekly retrospectives

  • Secure, HIPAA-compliant meal photo sharing, video chat visits, and text messaging
  • Personalized content delivery (meal plans, timely resources, daily tips)
